Organization

Visit our website www.healthwire.co and select the required category. Fill all the details like name of the organization, contact person’s name. You are supposed to upload the project in PDF format. The file must contain details like name of the organization and category you have opted. The document should include complete details of the project along with photographs. Companies have freedom to nominate their employees separately in Individual categories as well. Pay the financial contribution/fee and submit the project.

Individuals

• Individuals can nominate directly with one key reference of an authentic person or nominate by any third party in the select category.
• Nominee or their company and the third party who is nominating must provide their valid identity proof. The identity proof should either be Government IDs or IDs approved by the government. Also provide details of the contribution made by the nominated candidate which can be rewarded.
• Submit your details in a PDF format. No other formats will be accepted by us. Provide details like initiative, project handled, importance of the project, how many lives the project affected positively, areas and places etc. Documents containing feedback from people will be preferred.
• Supporting photographs, nominee’s video, video bite will be helpful in selection process.

COVID 19 EVALUATION PROCESS

• Authentic entries will be sent to the Eminent Advisory Board. Board members will nominate all the institutions and individual as per their nomination filed.
• We will check the authentication of the candidates and institutions without doubting their contribution.
• Nominations will be highlighted on our website along with their credentials and best of their contribution. Final acknowledgment and reward will be felicitated during a live Web Event on Healthwire Media’s Social Media Handle.
• The awards will be based on the nomination and all the categories will be evaluated as per the nomination filed by the institutions and nominated individuals.

Institutions

• Corporates/Businesses/ CSR Arms/Foundations : INR 15,000
• NGO/Religious/Philanthropy Organizations : INR 9,500
• Government Organizations : Complimentary

Individuals

•  Individual : INR 4500 (Including taxes)

• Amount mentioned is exclusive of  GST
• Amount mention is for all the categories
• Individual registration fee can be paid directly by nominee, company or third party reference
• For nominating group of individuals, Companies and organizations can nominate their prominent employees in a group categories.
